About The Position

The main purpose of this full-time position will be to act as a safety net, reducing the need for care at hospitals, inpatient facilities, detention centers, and other similar higher levels of care by managing people at the community level. While working alongside a licensed clinical crisis professional, the selected applicant will also engage individuals experiencing a crisis using a person-centered approach. This role requires home visits, community-based services, and on-call hours. The schedule includes evenings, weekends, and holidays based on program needs. The selected applicant must be willing and able to travel to all Worcester County Health Department sites.

Requirements

  • Graduation from an accredited high school or possession of a high school equivalency certificate.
  • One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
  • Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

  • Experience with assessments and diagnostic criteria.
  • Prior training associated with crisis work.

Responsibilities

  • Providing Naloxone education and training to individuals and families as appropriate.
  • Documenting notes and completing referrals.
  • Conducting follow-up visits and phone calls.
  • Identifying and reducing obstacles within the mental health system to support clients return to stability.
