About The Position

The main purpose of this full-time contractual position will be to provide community-based case management to individuals with intellectual/developmental disabilities. This position will assist eligible individuals and families in gaining access to resources in their community, planning for their future, education on transition services and waivers that could be available to them, as well as accessing needed services and supports. This position will also work with the individual and their identified team members to create the individual's person-centered plan. In addition, this position will work with the DDA regional office, local providers, staff and/or other community partners regularly. This position does include home visiting, community-based services and may require occasional evening and weekend hours. The selected applicant must be able to work and travel to all Worcester County Health Department sites.

Requirements

  • Graduation from an accredited high school or possession of a high school equivalency certificate.
  • One year of administrative staff or professional work in a health care setting.
  • A Bachelor's degree from an accredited college or university in a Human Services field; OR An Associates degree with two (2) years experience in a Human Services field; OR Seven (7) years experience in a Human Services field.

Nice To Haves

  • Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
