About The Position

This position will provide targeted case management services to eligible individuals with Developmental Disabilities who live in Cecil, Harford, and Kent Counties. This position will provide referrals for services, facilitate the development of the annual Person-Centered Plan, monitor individuals in service and on the Eligibility Waiting List, and assist in transition planning. This position requires the selected candidate to be highly organized with attention to detail. These efforts work to maintain the individual in the community and to promote individual choice and enhance overall quality of life.

Requirements

Graduation from an accredited high school or possession of a high school equivalency certificate.
One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

Bachelor’s degree in Human Services field.
