About The Position

ROCK is seeking a compassionate and highly organized Coordinated Service Planner to support children, youth, and families navigating complex needs. The ideal candidate will have a degree in a related health or social services field and at least three years of direct experience in child and youth mental health and developmental challenges. This role involves building collaborative relationships with families and community service providers to develop and implement family-centered Coordinated Care Plans. Responsibilities include facilitating service coordination meetings, navigating funding and referral processes, and advocating for resources. Strong communication, interpersonal skills, attention to detail, and proficiency in documentation and virtual platforms are essential. The role requires a commitment to culturally safe, respectful, and inclusive service planning, approaching work with integrity, empathy, and a desire to make a difference.

Requirements

  • A university degree in a related health care field or a Social Service/Child Youth Worker diploma.
  • At least three years of direct experience supporting children, youth, and families navigating complex and/or multiple special needs.
  • Deep understanding of child and youth mental health, developmental challenges, and the systems that support them.
  • Excellent communication skills, both written and oral.
  • Strong interpersonal skills.
  • Proficient in the use of technology and working in a virtual environment.
  • MAC OS operating systems.
  • Strong MS Word and Excel competencies.
  • Ability to work independently and as a team.
  • Reliable transportation for traveling within the Halton Region.
  • A valid Ontario License and access to use of a reliable vehicle.
  • Current (within 3 months) Criminal Reference Check (Vulnerable Sector Check, where working with the vulnerable population) and every 3 years.

  • First Aid/CPR-C Certification; required upon first 3 months of hire date.
  • Ability to delegate and guide collaboration within varieties of teams.

Responsibilities

  • Report to the manager for Coordinated Service planning.
  • Work collaboratively with families and community providers to support and develop a coordinated Care Plan that reflects the goals and needs of families with complex and/or multiple special needs.
  • Ensure the development of the plan and coordination of services and supports involved with families.
  • Collaborate with child/youth, families, and community partners to identify strengths and needs.
  • Gather information and determine appropriate level of support including funding, equipment, in-home safety or complex needs in collaboration with caregivers and service providers.
  • Initiate internal and external referrals in consultation with the family.
  • Facilitate and coordinate communication with service providers (family, schools, etc.) and clarify client needs.
  • Support development of a family centered collaborative coordinated service care plan with specific goals driven by the family.
  • Facilitate service coordination meetings with family and support team during service in accordance to the need of the family and planning, while ensuring the necessary service-providers are present to encourage community collaboration and knowledge transfer where applicable.
  • Provide timely Action Plans (Meeting Minutes) to family and attendees post- service coordination meetings.
  • Prepare appropriate documentation and present it to the Connections Halton Committee as needed.
  • Prepare appropriate documentation for Complex Special Needs. Funding resources and follow community processes for requesting OPR, out of area residential placements, and/or the use of CARS and CPRI assessment requests.
  • Collaborate closely with families and support teams to identify and connect with appropriate respite providers that align with the unique needs of the child/youth and their caregivers.
  • Regularly maintain and update respite tracking systems to ensure accuracy in billing and invoicing processes.
  • Schedule and coordinate appointments across internal teams, community partners, and families, ensuring seamless communication and time management.
  • Build and maintain strong, supportive relationships with clients and their families.
  • Provide specialized, dedicated, and at times urgent, resource coordination between multiple service providers, ensuring continuity and consistency in care.
  • Cultivate strong working relationships with external agencies and service providers, monitoring follow-through on all planned actions.
  • Complete all documentation, reporting, and clinical records promptly and in accordance with established standards.
  • Ensure the accuracy and integrity of case notes, statistics, and documentation relevant to the Coordinated Service Planner’s responsibilities.
  • Prioritize client safety by adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care.
  • Identify and report any client safety incidents.
  • Support client-centered care by adhering to ROCK’s principles.
