About The Position

The Connections Case Manager provides relationship‑based, trauma‑informed support to young people aged 16–25 who are navigating housing instability, and may also be experiencing system involvement, family conflict, or barriers to well‑being. This role focuses on early intervention, diversion, and coordinated support that strengthens natural supports, enhances stability, and builds the protective factors young people need to thrive, including building their formal and community supports. Working collaboratively across housing, systems, and community organizations, the Connections Worker helps youth identify goals, build trusted relationships, and access the resources required for long‑term success. The role includes hands‑on assistance, system navigation, and sustained advocacy to ensure youth receive responsive, consistent, and culturally safe support, to find appropriate and sustainable housing. This is a temporary, full-time position, working Monday to Friday, 37.5 hours per week, until April 1, 2027. This role works flexible hours, including evenings to support the staging, facilitation, and delivery of programs, and early mornings on a weekly basis to participate in community‑based outreach activities. The hourly wage range is $28.00 - $32.47 per hour dependent on experience and related skills. The successful candidate can expect to enter the organization no higher than the midpoint of the compensation range.

Requirements

  • Relevant degree in Social Work or Child & Youth Care or a related field (or equivalent experience).
  • Minimum of 2-3 years of experience working with vulnerable populations and at-risk youth.
  • Advanced knowledge of case management principles.
  • Demonstrated strong working knowledge of computer systems and applications, social media platforms, and technologies commonly used by youth.
  • ASIST and NVCI training.
  • Emergency First Aid certificate.
  • Experience with Microsoft Office applications including Outlook, Word, and Excel.
  • Valid driver’s license.
  • Current membership in relevant professional associations or societies.
  • Access to a reliable vehicle.
  • Able to provide up to date Immunization records.
  • Must have a Criminal Record Review Check completed by and satisfactory to Threshold.
  • Must sign Code of Conduct and Confidentiality Agreement.

  • Performs unit/room cleaning duties as required.
