Part-Time Concierge RN Nurse Care Manager
About The Position
We are a premier concierge private nursing in-home company dedicated to delivering bespoke, elevated care to private-pay clients. The company prioritizes support for its clinical team, capped caseloads, quality over volume, work-life balance, clinical autonomy, and unhurried, compassionate care. The RN Care Manager will work in a flexible job-share capacity with another experienced RN, dividing responsibility for client onboarding, quality oversight, and clinical coordination. The role focuses on high-touch personalized care, mentoring colleagues, and maintaining high clinical standards. Responsibilities include conducting in-home assessments for new clients, reviewing field staff documentation, leading client introductions and care transitions, coordinating with the paired RN, providing clinical oversight, and covering on-call nursing responsibilities one week per month.
Requirements
- Active, unencumbered Registered Nurse (RN) license in good standing.
- A minimum of 3–5 years of clinical nursing experience.
- Strong attention to detail and thorough understanding of clinical documentation standards and care planning.
- Exceptional interpersonal skills suited to a high-touch, private-pay clientele.
- Strong collaborative mindset suited to a job-share model and mentoring fellow nursing staff.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Experience in home health, private duty, care management, or concierge care preferred.
Responsibilities
- Conduct comprehensive, unhurried in-home assessments for new private-pay clients and establish personalized care plans.
- Audit and review nursing documentation, care logs, and assessments completed by field staff for clinical accuracy, compliance, and continuity.
- Lead client introductions and initial care transitions alongside field nurses, modeling best practices and supporting a smooth integration of care.
- Maintain active communication and seamless handoffs with the paired RN Care Manager to ensure client coverage and consistency.
- Serve as a clinical resource and advocate for clients, families, field nurses, and field caregivers, adjusting care plans as needs evolve.
- Cover on-call nursing responsibilities one week per month.
