Part-Time Concierge RN Nurse Care Manager
About The Position
We are seeking a detail-oriented, empathetic Registered Nurse (RN) Care Manager to join a premier concierge private nursing in-home company in a flexible job-share capacity. The role partners with another experienced RN to divide responsibility for client onboarding, quality oversight, and clinical coordination. It is suited to an experienced RN who enjoys personalized care, mentoring colleagues, and maintaining high clinical standards within a predictable, balanced schedule. The company provides bespoke, elevated care to private-pay clients. It caps caseloads and prioritizes quality, clinical autonomy, work-life balance, and unhurried, compassionate care. The position includes in-home assessments for new clients, review of field staff documentation, collaborative onboarding with field nurses, coordination and handoffs with the paired RN, and ongoing clinical oversight. The role also covers on-call nursing responsibilities one week per month.
Requirements
- Active, unencumbered Registered Nurse (RN) license in good standing.
- Minimum of 3–5 years of clinical nursing experience.
- Strong attention to detail and thorough understanding of clinical documentation standards and care planning.
- Exceptional interpersonal skills suited to high-touch, private-pay clientele.
- Strong collaborative mindset suited to a job-share model and mentoring nursing staff.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Background in home health, private duty, care management, or concierge care preferred.
Responsibilities
- Conduct comprehensive in-home assessments for new private-pay clients and establish personalized care plans.
- Audit and review nursing documentation, care logs, and assessments completed by field staff for accuracy, compliance, and continuity.
- Lead client introductions and initial care transitions alongside field nurses, modeling best practices and supporting smooth integration of care.
- Communicate and coordinate with the paired RN Care Manager to ensure seamless handoffs, coverage, and consistency for clients.
- Provide clinical guidance and advocacy for clients, families, field nurses, and field caregivers, adjusting care plans as needs evolve.
- Cover on-call nursing responsibilities one week per month.
- Mentor fellow nursing staff and collaborate in the job-share model.
