About The Position

Cheer Health Care is hiring a Registered Nurse to provide bespoke, one-on-one home health services to seniors in La Jolla and North San Diego County, California. In a concierge capacity, the nurse partners with an interdisciplinary care team to design, carry out, and monitor personalized health programs, acting as a trusted clinical advocate and communication hub for clients, family members, and clinicians. The role emphasizes unhurried, patient-first care, meaningful relationships, empathy, and supporting seniors’ comfort and quality of life at home.

Requirements

Current, unrestricted California Registered Nurse license.
Three years of experience as a registered nurse.
Proof of a negative TB result.
Physical screening documentation from within the last year.
Active California driver’s license and car insurance.
Cell phone with internet access.

Nice To Haves

Home health experience preferred.

Responsibilities

Conduct detailed clinical evaluations of patients’ physical condition, psychological well-being, and living environment.
Develop and coordinate dynamic, evidence-driven care plans with concrete health milestones tailored to each patient’s lifestyle and goals.
Direct long-term chronic illness management strategies, emphasizing preventive care to help avoid hospital readmissions and health declines.
Provide nursing interventions, including surgical aftercare, complex wound dressing, and medication administration.
Coach patients and designated caregivers on diagnoses, recovery protocols, and everyday self-management.
Promote senior independence, fall prevention, safety, and overall well-being at home.
