About The Position

Cheer Health Care is hiring a Registered Nurse to provide bespoke, one-on-one home health services to seniors in the La Jolla and North San Diego County communities. In this concierge role, the RN partners with an interdisciplinary care team to design, deliver, and monitor personalized health programs, acting as a clinical advocate and communication hub for clients, families, and clinicians. The position is described as per diem, with 12-hour day shifts. The posting lists pay as $45–$80 per hour depending on experience and later states $48–$75 per hour based on experience, with potential for a team bonus.

Requirements

Current, unrestricted California registered nurse license.
Three years of experience as a registered nurse.
Proof of a negative TB result.
Physical screening documentation from within the last year.
Active California driver’s license and car insurance.
Cell phone with internet access.

Responsibilities

Perform detailed clinical evaluations of patients’ physical condition, psychological well-being, and living environment.
Develop evidence-driven, individualized care plans with concrete health milestones tailored to each patient’s lifestyle and goals.
Direct long-term chronic illness management strategies, emphasizing preventive care to help avoid hospital readmissions and health declines.
Provide nursing interventions including surgical aftercare, complex wound dressing, and medication administration.
Coach patients and designated caregivers on diagnoses, recovery protocols, and everyday self-management.
Promote senior autonomy, fall prevention, safety, and well-being to maximize comfort and quality of life at home.
Coordinate with clients, family members, and fellow clinicians as a trusted clinical advocate and communication hub.
