About The Position

Requirements

Current, unrestricted California registered nurse license.

Responsibilities

Perform detailed clinical evaluations of patients’ physical condition, psychological well-being, and living environment.
Develop dynamic, evidence-driven care plans with concrete health milestones tailored to each patient’s lifestyle and goals.
Direct long-term chronic disease management strategies and emphasize preventive care to help avoid hospital readmissions and health declines.
Provide nursing procedures including surgical aftercare, complex wound dressing, and medication administration.
Coach patients and designated caregivers on diagnoses, recovery protocols, and everyday self-management.
Promote senior independence, fall prevention, safety, and quality of life at home.
Partner with an interdisciplinary care team and serve as a clinical advocate and communication hub for clients, family members, and fellow clinicians.
