We deliver trauma-informed, whole-person care that includes follow-up medical attention, mental health support, substance use recovery, life skills development, and assistance securing IDs, benefits, and housing. The Community Support Specialist provides person-centered support and care plan execution during daytime hours, when clients are most active and engaged. This role ensures client safety and well-being through direct, hands-on support with daily living activities, appointments, and program participation, while maintaining clear and consistent professional boundaries. The Community Support Specialist supports client progress toward housing stability and health outcomes by executing care plans, actively engaging clients throughout the day, helping them access services in the community, and completing accurate documentation to ensure continuity of care across all shifts. The ideal candidate is dependable, attentive, calm under pressure, and able to remain alert throughout the shift.
