The Community Support Specialist is responsible for providing person-centered support and care plan execution during overnight hours. This role ensures client safety and well-being through daily welfare checks, observation of behavioral changes, and engagement techniques. The Community Support Specialist supports client progress toward housing stability and health outcomes by executing care plans, maintaining a safe and stable environment, and completing accurate documentation to ensure continuity of care. The ideal candidate is dependable, attentive, calm under pressure, and able to remain alert throughout the overnight shift.
