Requirements

Bachelor's Degree from an accredited college or university in a mental health-related field or A Licensed Registered Nurse with related experiences or A Licensed Practical Nurse with two years of related work or life experience
Knowledge of community resources and agencies
Possession of a valid New Jersey driver's license with a clean five-year driving record that meets organizational standards and qualifies for insurance coverage.
Willingness to work a flexible 40-hour schedule which may include evenings weekends holidays and on-call rotation

Responsibilities

Advocacy and support for individuals with mental illness living in the community.
Help clients secure housing and maintain their integration into the community, fostering their self-sufficiency and independence.
Promoting the use of vital community services.
Supporting clients in travel and fiscal management.
Facilitating access to healthcare.
Collaboration with treatment providers.
Develop tailored Individual Rehabilitation Plans.
Documenting progress to ensure effective service delivery.
Assisting individuals in honing their social skills and daily living capabilities.
