Requirements
- Bachelor's Degree from an accredited college or university in a mental health-related field or A Licensed Registered Nurse with related experiences or A Licensed Practical Nurse with two years of related work or life experience
- Knowledge of community resources and agencies
- Possession of a valid New Jersey driver's license with a clean five-year driving record that meets organizational standards and qualifies for insurance coverage.
- Willingness to work a flexible 40-hour schedule which may include evenings weekends holidays and on-call rotation
Responsibilities
- Help clients secure housing and maintain their integration into the community, fostering their self-sufficiency and independence.
- Promote the use of vital community services.
- Support clients in travel and fiscal management.
- Facilitate access to healthcare.
- Collaborate with treatment providers to develop tailored Individual Rehabilitation Plans.
- Document progress to ensure effective service delivery.
- Assist individuals in honing their social skills and daily living capabilities.
