Requirements

  • Bachelor's Degree from an accredited college or university in a mental health-related field or A Licensed Registered Nurse with related experiences or A Licensed Practical Nurse with two years of related work or life experience
  • Knowledge of community resources and agencies
  • Possession of a valid New Jersey driver's license with a clean five-year driving record that meets organizational standards and qualifies for insurance coverage.
  • Willingness to work a flexible 40-hour schedule which may include evenings weekends holidays and on-call rotation

Responsibilities

  • Help clients secure housing and maintain their integration into the community, fostering their self-sufficiency and independence.
  • Promote the use of vital community services.
  • Support clients in travel and fiscal management.
  • Facilitate access to healthcare.
  • Collaborate with treatment providers to develop tailored Individual Rehabilitation Plans.
  • Document progress to ensure effective service delivery.
  • Assist individuals in honing their social skills and daily living capabilities.
