This position description is a temporary management guide tool and is subject to change. The Community Case Manager provides competent community-based services in the community, school, and patient’s home, as assigned by supervisor and as specified in the patient’s treatment plan. Services are based on the philosophies and principles of emphasizing patient’s strengths. The role involves assisting staff in identifying and establishing rapport with new community-based services patients, communicating with referral sources, and providing case consultation. The position also requires maintaining clinical outcomes, providing services in Prairie View’s Summer Intensive Rehabilitation Program (SIRP), and adhering to ethical standards. Key activities include highlighting patient/family strengths, employing problem-solving, providing crisis intervention, assessing strengths and setting goals, linking patients with community services, teaching life and parenting skills, encouraging community involvement, and providing support and advocacy. Flexibility in providing services to meet patient needs is essential. The role requires demonstrating competency in working with child/adolescent patients and their caregivers, understanding child/adolescent behavior and environmental structures, establishing appropriate limits and boundaries, and understanding risk factors and interventions for patient safety. If participating in psychosocial group work, knowledge of group management strategies and curriculum development is needed. Administrative responsibilities include attending meetings, maintaining personal records, timely documentation, working cooperatively with staff, participating in supervision, organizing schedules, and working with Utilization Review staff to ensure authorizations and treatment plans are in place. Medical records, including progress notes and monthly CSR information, must be completed according to standards. The role also involves meeting monthly billable expectations, transporting patients, providing community education, and engaging in professional development.
