Responsibilities often include helping participants access housing, transportation, food assistance, utility support, and public benefits such as Medicaid and Medicare. The specialist also works closely with nurses, social workers, therapists, and other members of the interdisciplinary PACE team to coordinate care and advocate for participants’ needs. In addition to resource coordination, the position involves educating families about available services, maintaining participant documentation, and providing emotional support during challenging situations. Overall, the goal of the role is to improve quality of life for elderly participants while helping them avoid unnecessary nursing home placement through comprehensive community-based support.
