About The Position

As a Community Resource Specialist (CRS) at One Health, you’ll be a frontline advocate for patients, connecting them with vital resources and support systems. This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing. If you thrive in a fast-paced, collaborative environment and have a heart for service, this is your chance to make a real difference! This is a full-time role working 40 hours per week, with a current schedule of Monday, Tuesday, Thursday from 7:00 AM to 5:00 PM and Wednesday from 7:00 AM to 7:00 PM, with the possibility of future schedule changes to include Friday hours.

Requirements

Strong computer and documentation skills
Ability to manage multiple priorities with remote supervision
Professionalism, empathy, and clear boundaries
Community involvement or connection to the population served
High School Diploma or GED
Valid Driver’s License

Responsibilities

Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services.
Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
Educate & Advocate: Assist with applications for coverage, benefits, and other essential services.
Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans.
Engage the Community: Participate in outreach events and consortiums to promote One Health services.
Navigate Resources: Stay current on eligibility rules, insurance options, and enrollment procedures.
