About The Position

The Community Resource Specialist I provides high-quality customer service and serves as the main point of contact for Neighborhood Outreach Access to Health (NOAH). The role supports patients by sharing information about NOAH services and community resources, connecting them with appropriate assistance, and helping resolve concerns. NOAH is a Federally Qualified Health Center offering comprehensive, integrated, and affordable healthcare services to people in need, including medical, dental, behavioral health, nutrition, preventive health, eligibility assistance, and health education programs.

Requirements

High school diploma or GED.
At least 1 year of customer service experience.
Healthcare industry experience.
Excellent communication skills, including active listening.
Service orientation and ability to resolve patient grievances.
Proficient computer skills and ability to learn new software.
Knowledge of, or ability to learn, basic medical terminology and NOAH services.
Pass a pre-employment background check and drug testing.
Have a current Level 1 Fingerprint Clearance Card or pass fingerprint clearance within 7 days following hire.

Responsibilities

Schedule and confirm appointments and verify insurance.
Manage referrals, work queues, patient lists, emails, and referrals in the Electronic Health Record.
Interact with patients by telephone, email, or in person to provide support and information about NOAH services.
Explain community resources and connect patients with appropriate resources.
Ensure appropriate actions are taken to resolve patient problems and concerns.
Promote and provide access to care.
Attend, participate in, or assist with meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.
Perform other related duties as assigned.
