Requirements

  • Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, or a related field; or equivalent experience in case management or care coordination.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong organizational skills with the ability to manage multiple priorities.
  • Valid Connecticut driver’s license and reliable transportation.

Nice To Haves

  • Experience in healthcare, community-based services, or social services.
  • Familiarity with Epic or other electronic health record systems.
  • Bilingual English/Spanish proficiency.
  • Community Health Worker (CHW) certification or willingness to obtain certification.

Responsibilities

  • Conduct comprehensive needs assessments to identify health, social, financial, and community support needs.
  • Partner with clients to develop individualized service plans that address their unique goals and challenges.
  • Provide ongoing support, advocacy, and follow-up to help clients achieve positive outcomes.
  • Assist clients with accessing and enrolling in public benefit programs such as SNAP, Medicaid, Medicare, SSI/SSDI, and other entitlement services.
  • Connect clients to healthcare, housing, food assistance, transportation, behavioral health, and other community resources.
  • Build strong relationships with community partners to ensure seamless referrals and coordinated care.
  • Serve as a trusted resource and advocate for clients navigating complex systems.
  • Collaborate with healthcare providers, social service agencies, and multidisciplinary teams to improve client outcomes.
  • Promote dignity, respect, and equitable access to services for all individuals and families.
  • Document client interactions, referrals, and outcomes accurately in electronic platforms.
  • Track program metrics and support reporting requirements.
  • Maintain strict confidentiality and compliance with HIPAA, insurance, and organizational policies.
  • Utilize electronic health records, case management systems, and digital communication tools effectively.
  • Bring creativity, initiative, and problem-solving skills to address client needs and improve service delivery.
  • Continuously learn and adapt in a dynamic and collaborative environment.
