About The Position

Aspen Hope Center (AHC) provides compassionate mental health and crisis support to individuals and families across the Roaring Fork and Colorado River Valleys. We are committed to helping people access care, resources, and support in a respectful and welcoming environment. The Community Resource Navigator, also known as a Community Health Worker, helps people connect with mental health, behavioral health, and community resources. This role supports both Spanish and English-speaking individuals and families, including people who may not already be clients of Aspen Hope Center. The Navigator helps community members understand available services, schedule appointments, connect with providers, and overcome barriers to care. This position works closely with schools, healthcare providers, and community organizations to make sure people receive the support they need. This role is ideal for someone who enjoys helping others, communicating with people from many backgrounds, staying organized, and building strong community relationships.

Requirements

  • High school diploma or equivalent
  • Fluent in both Spanish and English (spoken and written)
  • Strong communication and customer service skills
  • Experience working with diverse populations and building positive, professional relationships
  • Basic computer and data entry skills
  • Ability to stay organized, work independently, and maintain confidentiality

Responsibilities

  • Serves as a trusted bilingual (Spanish-English) support person for community members seeking mental health or behavioral health services
  • Helps individuals identify their needs and connect them with the right resources and services
  • Assists clients with scheduling AHC therapy appointments (using Simple Practice) and follow up coordination
  • Helps therapy clients connect with therapists based on availability and areas of specialty
  • Informs AHC clients of payment options and provides portal assistance for clients as needed
  • Explains payment options and assists clients with online forms or client portals when needed
  • Conducts regular audits of therapy client paperwork and payments; follows up as needed
  • Supports applications for the Community Hope Fund and assists with tracking related information
  • Completes Community Health Worker training and credentialing through the Colorado Department of Public Health and Environment (CDPHE)
  • Helps individuals understand referrals, care plans, and available services
  • Coordinates referrals and “warm handoffs” between mental health, medical, school, substance use, and community services
  • Works with schools, healthcare providers, and community organizations to improve access to care
  • Builds positive relationships with community partners and helps reduce cultural, language, and financial barriers
  • Maintains an updated list of local resources, including immigrant-friendly services
  • Helps create bilingual outreach and educational materials
  • Increases awareness of mental health and community resources throughout the region
  • Answers phones professionally and directs calls to the appropriate staff member
  • Responds to general email inquiries
  • Welcomes visitors and helps connect them with the correct staff member or service
  • Helps maintain an organized and welcoming front desk and reception area
  • Assists with reviewing client paperwork and payments for accuracy
  • Ensures therapy and crisis paperwork is organized and available when needed
  • Completes documentation and data entry accurately and on time
  • Performs other duties as assigned
