About The Position

Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. Responsible for providing direct support and supervision to individuals residing in community-based behavioral health or supportive living programs. The Community Residence Counselor assists residents in developing daily living skills, achieving treatment goals, and maintaining a safe and supportive environment. This role plays a vital part in promoting recovery, independence, and quality of life for individuals with mental health, substance use, or developmental conditions. These programs serve individuals with moderate to severe or treatment-resistant OCDI servicing adults 18 and older on our Belmont campus.

Requirements

  • High School Diploma or Equivalent required
  • Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
  • Time Management.
  • Excellent verbal and written communication skills.
  • Ability to work as part of a team.
  • Maintains a commitment to professional growth and competency.
  • All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.

Nice To Haves

  • Bachelor's Degree Related Field of Study preferred
  • One-year patient care or related experience preferred, preferably in a psychiatric setting
  • 1-2 years preferred
  • Familiarity with Epic preferred.

Responsibilities

  • Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities according to needs. This may include driving & transporting patients to and from activities in hospital vehicles.
  • Remains awake and alert, at all times, while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourage patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Physical ability for participation in patient activities.
  • Performs other duties as assigned
