About The Position

Under direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. This role involves establishing helpful relationships with patients while maintaining professional boundaries, participating in patient rounds and status checks, and completing assigned patient care tasks. The CRC works effectively to manage the therapeutic milieu, provides patients with direction and supervision, and follows established hospital and program policies, procedures, and protocols. Communication of patient milieu information and observations related to medication side effects or patient changes to appropriate staff is crucial. The role also includes encouraging patients in their activities of daily living, assessing sleep patterns, completing mandatory annual in-service requirements (including CPR and de-escalation), and attending rounds and case conferences. The CRC supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed. This is a 40-hour per week position.

Requirements

  • High School Diploma or Equivalent required
  • Basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
  • Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required

Nice To Haves

  • Bachelor's Degree Related Field of Study preferred
  • Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
  • One-year patient care or related experience preferred, preferably in a psychiatric setting
  • Familiarity with Epic preferred

Responsibilities

  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourage patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.
  • Other duties as assigned.
