About The Position

Under direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. This role is crucial for supporting patients in a community residence setting, assisting with daily routines, medical reminders, meal preparation, transportation, and personal care as needed. The CRC is expected to maintain a therapeutic environment, establish helpful relationships with patients while maintaining professional boundaries, and contribute to patient care through clear documentation and communication.

Requirements

High School Diploma or Equivalent required
Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.

Nice To Haves

Bachelor's Degree Related Field of Study preferred
Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
One-year patient care or related experience preferred, preferably in a psychiatric setting
1-2 years preferred
Familiarity with Epic preferred.

Responsibilities

Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
Provides and supervises community activities according to needs.
May include driving & transporting patients to and from activities in hospital vehicles.
Remains awake and alert, at all times, while on the job.
Assists patients in maintaining and keeping various appointments.
Clear, concise, and timely documentation and shift notes.
Establishes relationships with patients that are helpful and maintains professional boundaries.
Participates in patient rounds, status checks, or teams per program guidelines.
Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
Works effectively to manage therapeutic milieu.
Provides patients with direction and supervision as needed.
Follows established hospital & program policies, procedures, and protocols.
Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
Encourage patients in their activities of daily living.
Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
Attends and participates in rounds and case conferences when appropriate.
Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.
Other duties as assigned.
