About The Position

Under the direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in the therapeutic milieu within the scope of defined practice. This role is essential for supporting individuals with moderate to severe or treatment-resistant Obsessive-Compulsive Disorder (OCD) and related conditions, servicing adults aged 18 and older on the Belmont campus. The CRC is responsible for maintaining a safe and therapeutic environment, assisting patients with daily living activities, and ensuring timely and accurate documentation.

Requirements

  • High School Diploma or Equivalent required
  • Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
  • Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
  • One-year patient care or related experience preferred, preferably in a psychiatric setting
  • 1-2 years preferred
  • Basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
  • All employees must possess basic computer skills.

Nice To Haves

  • Bachelor's Degree Related Field of Study preferred
  • Familiarity with Epic preferred

Responsibilities

  • Provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities.
  • May include driving and transporting patients to and from activities in hospital vehicles.
  • Remains awake and alert at all times while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Completes clear, concise, and timely documentation and shift notes.
  • Establishes helpful relationships with patients and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourages patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles, notifying the clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Physical ability for participation in patient activities.
  • Other duties as assigned.
