About The Position

Under the direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. This role serves individuals with moderate to severe or treatment-resistant OCDI, servicing adults 18 and older on the Belmont campus. The CRC is responsible for assisting patients with daily routines, medical reminders, meal preparation, transportation, and personal care as needed. They must remain awake and alert at all times, document clearly and concisely, and establish helpful, professional relationships with patients. The position involves participating in patient rounds, completing assigned patient care tasks, managing the therapeutic milieu, and communicating observations to the clinical team.

Requirements

  • High School Diploma or Equivalent required
  • Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
  • Basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
  • Ability to drive, including transporting patients, may be required based on role and location.

Nice To Haves

  • Bachelor's Degree Related Field of Study preferred
  • Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
  • One-year patient care or related experience preferred, preferably in a psychiatric setting
  • Familiarity with Epic preferred

Responsibilities

  • Provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities according to needs.
  • Remains awake and alert, at all times, while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourage patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.
