About The Position

Under direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. This role provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse. The CRC provides and supervises community activities according to needs, remains awake and alert at all times, and assists patients in maintaining and keeping various appointments. The role requires clear, concise, and timely documentation and shift notes, and establishing relationships with patients that are helpful while maintaining professional boundaries. The CRC participates in patient rounds, status checks, or teams per program guidelines, and completes assigned patient care tasks per program guidelines and/or daily assignment sheet. The role involves working effectively to manage the therapeutic milieu, providing patients with direction and supervision as needed, and following established hospital & program policies, procedures, and protocols. Communication of information regarding patient milieu/therapeutic environment and observations related to medication side effects, physical and emotional changes to appropriate staff is crucial. The CRC encourages patients in their activities of daily living, assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary. The role requires completing mandatory annual in-service requirements, including CPR and de-escalation, and attending and participating in rounds and case conferences when appropriate. Physical ability for participation in patient activities is necessary. The CRC supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.

Requirements

High School Diploma or Equivalent required
Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
Ability to drive, including transporting patients, may be required based on role and location.

Nice To Haves

Bachelor's Degree Related Field of Study preferred
Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
One-year patient care or related experience preferred, preferably in a psychiatric setting
1-2 years preferred
Familiarity with Epic preferred
