About The Position

Appleton is a step-down residential program for adults with serious mental illness, designed to help them stabilize, build skills, and transition back to independent living. Under the direction of the Program Manager, the Community Residence Counselor provides direct patient care and participates in the therapeutic milieu within the scope of defined practice.

Requirements

High School Diploma or Equivalent required
Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required
All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.

Nice To Haves

Bachelor's Degree Related Field of Study preferred
Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred
One-year patient care or related experience preferred, preferably in a psychiatric setting
1-2 years preferred
Familiarity with Epic preferred.
Ability to drive, including transporting patients, may be required based on role and location.

Responsibilities

Provides direct patient care in accordance with the current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
Provides and supervises community activities according to patient needs.
Remains awake and alert at all times while on the job.
Assists patients in maintaining and keeping various appointments.
Completes clear, concise, and timely documentation and shift notes.
Establishes helpful relationships with patients while maintaining professional boundaries.
Participates in patient rounds, status checks, or teams per program guidelines.
Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
Works effectively to manage the therapeutic milieu.
Provides patients with direction and supervision as needed.
Follows established hospital and program policies, procedures, and protocols.
Communicates information regarding the patient milieu/therapeutic environment to appropriate staff in a timely manner.
Communicates observations related to medication side effects and physical and emotional changes to appropriate medical staff in a timely manner.
Encourages patients in their activities of daily living.
Assesses sleep patterns and assists patients in developing more appropriate sleep cycles, notifying the clinical team as necessary.
Completes mandatory annual in-service requirements, including but not limited to CPR and de-escalation.
Attends and participates in rounds and case conferences when appropriate.
Maintains the physical ability to participate in patient activities.
Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.
