The Community Relations Liaison plays a vital role in advancing awareness, understanding, and utilization of Supportive Care service lines. This position is responsible for providing education and outreach to community members, physicians, hospitals, health care providers, and organizational partners regarding the full continuum of services, including Home Health, Hospice, Palliative Care, Connected Care Remote Patient Monitoring, Mobile Health, Advanced Wound Healing, and other Supportive Care programs as assigned. The Liaison functions as a dynamic representative of the organization, serving as both an educator and relationship-builder. Through strategic outreach, effective communication, and trusted collaboration, the Liaison ensures that referral sources are well-informed, patient transitions are seamless, and the community has access to high-quality, compassionate care. In addition, the Liaison is accountable for identifying new business opportunities, tracking referral patterns, and implementing targeted growth initiatives that align with the organization’s strategic priorities. By blending community relations, partnership development, and business acumen, the Liaison drives sustainable growth for the service line while enhancing the organization’s visibility, reputation, and impact in the community.
