About The Position

Make a difference in the lives of Brooklyn's residents, neighborhoods, and communities. For over 50 years PSS has performed vital and important work -- through its ten centers, two apartment buildings and affiliated programs. The agency touches on the lives of over 2,000 people each day. As the Social Worker, you will be responsible for coordinating supportive services for residents in one of the many communities that PSS serves. The Social Worker’s role is vital in ensuring effective supportive services are put in place that allows the residents to remain independent, active and to thrive in their home and community. New York Connects: NY Connects Brooklyn is a borough-wide program funded by NYC Aging and administered in partnership with the New York State Office for the Aging (NYSOFA). The program provides free, unbiased, and comprehensive Information and Assistance (IA) on long-term services and support (LTSS) for older adults, people with disabilities, caregivers, and professionals. The Community Outreach Coordinator is responsible for expanding community awareness of NY Connects Brooklyn through outreach, public education, and partnership development. This position coordinates community engagement activities, delivers educational presentations, builds relationships with community organizations, and ensures Outreach and Public Education activities are fully documented and audit-ready in accordance with NYC Aging requirements. The ideal candidate is passionate about aging and disability equity, community engagement, and connecting historically underserved communities to essential resources and services. Requirements

Bachelor’s degree in human services, Social Work, Public Health, Community Health, Gerontology, Sociology, Psychology, Communications, Public Administration, Nonprofit Management, Community Development, or a related field.
Minimum of two years of experience in community outreach, community engagement, public education, nonprofit programming, health promotion, or human services.
Excellent interpersonal, presentation, and relationship-building skills.
Strong organizational skills with the ability to manage multiple projects and maintain accurate documentation.
Proficiency in Microsoft Office and Google Workspace.
Ability to travel throughout Brooklyn and attend occasional evening or weekend events.
Committed to aging and disability equity, inclusion, and empowerment.
Culturally responsive and community minded.
Highly detail-oriented with strong documentation and organizational skills.
Comfortable balancing community-based work with administrative responsibilities.
Self-directed and proactive, with the ability to multitask in a fast-paced program environment.
Ability to travel independently throughout Brooklyn using public transportation/ ride-share.

Responsibilities

Plan, coordinate, and conduct outreach activities throughout Brooklyn, including community events, health fairs, tabling events, flyering, and informational sessions.
Develop and maintain partnerships with senior centers, hospitals, community-based organizations, libraries, faith-based organizations, elected officials' offices, NYCHA developments, and other community stakeholders.
Identify opportunities to expand awareness of NY Connects and increase cross-referrals between programs and partners.
Represent NY Connects Brooklyn at community meetings and networking events while serving as an ambassador for the program.
Collaborate with community partners to coordinate outreach events and promote available long-term services and supports.
Coordinate and facilitate public education presentations on topics approved by NYC Aging, including long-term services and supports, public benefits, caregiving resources, housing, and healthy aging.
Tailor presentations to meet the needs of diverse audiences, including older adults, caregivers, professionals, and community organizations.
Distribute educational materials and respond to questions about NY Connects services and available community resources.
Track attendance and evaluate outreach efforts to identify opportunities for future programming.
Complete timely and accurate documentation for all Outreach and Public Education activities in accordance with NYC Aging requirements.
Maintain organized records of outreach activities, including new partner information, event details, sign-in sheets, flyers, presentation materials, attendance records, photographs (when appropriate), and event correspondence.
Track and identify areas of potential growth, including geographic areas, populations served, and types of organizations engaged.
Create and maintain the Outreach and Public Education compliance binder to ensure documentation is complete, organized, and audit-ready throughout the contract year.
Assist with tracking monthly Outreach and Public Education deliverables and contribute success stories and program highlights for monthly reports.
Work closely with the Program Director to ensure outreach activities meet contractual requirements and quality assurance standards.
Participate in staff meetings, supervision, training, and strategic planning activities.
Assist with special outreach campaigns, community awareness initiatives, and other projects that support program goals.
Maintain an outreach calendar and communicate upcoming events and partnership opportunities with the program team.
Perform other duties as assigned in support of NY Connects Brooklyn.
