Responsibilities

  • Community Outreach & Relationship Building Conduct regular in-person visits to: Hospitals Senior living communities Rehab and skilled nursing facilities Case managers and community partners Build and maintain trusted, professional relationships Represent Healthy at Home Caregivers consistently and professionally Stay visible and active in the community
  • Client Growth & Revenue Generation Generate new client opportunities through outreach efforts Convert referrals into active care cases Drive weekly growth in billable hours Focus on high-quality referral sources that produce real results
  • Pipeline Ownership & Follow-Through Track all outreach, referrals, and follow-ups in CRM Ensure no qualified lead is lost due to lack of follow-up Maintain an active and organized pipeline Follow each opportunity through to conversion or closure
  • Reporting & Accountability Provide weekly reporting on: Referrals generated New clients started Billable hours added Take ownership of performance metrics and outcomes
