About The Position

The Evergreen Center is seeking an experienced Licensed Practical Nurse (LPN) to provide nursing care at its adult support residences. The nurse carries out routine health care and first-aid procedures, maintains medical records, and assesses emergency injuries or illnesses and informs the Director of Health Services. The role is part-time at 20 hours per week, with schedule flexibility, and reports directly to the Director of Health Services.

Requirements

• Current Massachusetts LPN license.
• At least two years of LPN experience.
• Experience working with people who have developmental disabilities.
• Current malpractice insurance.

Responsibilities

• Identify individuals’ health care needs and participate in developing, implementing, and revising Health Care Plans.
• Monitor health and safety issues within program locations offering residential services.
• Provide initial orientation and ongoing in-service training to staff on health and safety needs, medical conditions, diagnoses, and treatment plans.
• Train and provide ongoing supervision to staff in medication administration.
• Carry out routine health care and first-aid procedures.
• Maintain medical records.
• Assess emergency injuries or illnesses and inform the Director of Health Services.
