About The Position

Community Vision is looking for a Community Living Coordinator to join their team. This role supervises direct support staff and works one-on-one with individuals in the Community Living program. The coordinator provides direct support, manages staff schedules, and facilitates activities to ensure individuals have opportunities both at home and in the community. The ideal candidate will have a trauma-informed approach, be highly organized, adaptable, and able to quickly establish healthy connections with individuals and their natural support systems.

Requirements

  • At least 3 years of paid experience in the developmental disabilities field, or in human services.
  • At least 1 year of paid experience managing others; preferably with experience coordinating schedules.
  • Frequent driving is required for this position. Candidates must be available/able to reliably travel to various locations within a 20 mile radius of our SE Portland Office.
  • Auto liability insurance must be carried through the duration of employment and a clean driving record with a valid Driver’s license is required.
  • Must be able to push, pull, and lift up to 50 lbs.
  • Must be able to obtain approved Criminal History Check through the Oregon Department of Human Services Background Check Unit prior to working independently, without direct supervision.
  • Must not be on the Office of Inspector General's (OIG) List of Excluded Individuals.
  • Must maintain effective CPR/First Aid Certification and OIS Certification (on-site, paid training provided by CV).

Responsibilities

  • Completes administrative and managerial duties including supporting program development, coaching and supervising direct support staff, and completing accurate documentation.
  • Provides individualized direct support, which may include financial management, medical management, grocery shopping, community activities, household maintenance, and creating and maintaining relationships through social skill development.
  • Works with the individual and their support team, including families, in identifying and monitoring specific activities to achieve goals in their Individual Support Plan, according to the Service Agreement.
