About The Position

The Community Liaison serves as the primary relationship manager between Enhabit and referral sources, including physicians, physician practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations and professionals. The role supports referral growth by raising awareness of company services and explaining program requirements to referral sources, patients, families, caregivers, and community partners. This work helps people navigate post-acute care transitions, with the goals of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. The position requires a bachelor's degree in healthcare administration, marketing, or a related field, or graduation from an approved school of nursing, therapy, or social work. Equivalent years of work experience may be considered in lieu of degree requirements. Candidates must have at least two years of experience in healthcare business development, community relations, marketing, or sales, and demonstrated understanding of laws and regulatory guidelines governing Medicare-certified home health and hospice operations. The role requires a valid driver's license, automobile liability insurance, dependable transportation, and the ability to drive safely in all weather conditions. Strong interpersonal, communication, and presentation skills, English fluency, active listening, and intermediate technology skills—especially Microsoft Word and Excel—are also required. Clinical candidates must maintain a clinical license as required by state rules. Except for designated full-time remote positions, in-office attendance is essential to support supervision, teamwork, task completion, and other position objectives. Occasional remote work to accommodate temporary personal needs or crises does not waive this essential function.
