Enhabit Home Health & Hospice•Austin, TX

About The Position

Enhabit Home Health & Hospice is seeking a Community Liaison Home Health to serve as the primary relationship manager between the company and referral sources. The role supports referral growth by educating referral sources, patients, families, caregivers, and community partners about company services and program requirements. It helps people navigate post-acute care transitions, with the goals of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. The position requires a relevant bachelor’s degree or graduation from an approved school of nursing, therapy, or social work; equivalent work experience may substitute for degree requirements. Candidates need at least two years of experience in healthcare business development, community relations, marketing, or sales, as well as demonstrated knowledge of laws and regulatory guidelines governing Medicare-certified home health and hospice operations. The role also requires a valid driver license, automobile liability insurance, dependable transportation, and the ability to drive safely in all weather conditions. For clinical candidates, an active clinical license is required as applicable to state requirements. Unless designated as a full-time remote, the position requires in-office attendance to support teamwork, supervision, task completion, and other objectives. Occasional remote work to accommodate temporary personal needs or crises does not waive this essential function.
