About The Position

Enhabit Home Health & Hospice is seeking a Community Liaison Home Health to serve as the primary relationship manager between the company and referral sources. The role supports referral growth by raising awareness of the company’s services and program requirements among referral sources, patients, families, caregivers, and community partners. It helps people navigate post-acute care transitions, with the goals of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. The role requires at least two years of relevant experience, knowledge of laws and regulations governing Medicare-certified home health and hospice operations, a valid driver’s license and automobile liability insurance, dependable transportation, and strong communication and technology skills. In-office attendance is essential except for designated full-time remote positions; occasional remote work to accommodate temporary personal needs or crises does not waive this function. Enhabit describes itself as one of the nation’s largest home-based care providers and highlights organizational stability, a collaborative culture, professional growth, and ongoing development. It is an equal opportunity employer committed to a collaborative and respectful work environment without discrimination on protected grounds.
