About The Position

Trillium Health Resources is seeking a Community Liaison Coordinator to join their team. This role is responsible for the overall coordination and oversight of developing and supporting a community for all members receiving services within assigned regions of the Trillium MCO/Local Management Entity catchment area. The Coordinator will assist with program development, consultation, resource management, and coordination with other services and agencies to best meet the needs of individuals served by various stakeholders in the community. The position requires frequent travel within the assigned region and may involve working in other regions, after hours, and on weekends occasionally. The goal is to help individuals and families build strong foundations for healthy, fulfilling lives by making a tangible impact.

Requirements

  • High School Diploma/GED and six (6) years’ experience working with individuals with Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR Associate’s degree and four (4) years’ experience working with individuals with Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR Bachelor’s degree and a minimum of two (2) years’ experience working with individuals with Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Northern Region, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, and Warren.
  • Must be able to travel within catchment as required.

Nice To Haves

  • Master’s degree in Human Services field
  • Four (4) years of the above experience
  • Experience working with community-based stakeholders

Responsibilities

  • Coordinate and/or support stakeholder meetings and events held in the community or virtually (frequent travel is required).
  • Attend staff meetings of partner agencies to explain changes and updates in the Behavioral Health and I/DD system as well as promote best practices as a Tailored Plan.
  • Educate other employees and the community at large about services, changes, funding source requirements, eligibility, and service availability.
  • Work with provider agencies and community stakeholders to encourage the whole person care approach to physical and behavioral health services, person-centered planning process, and provide technical assistance as needed.
