About The Position

The Community Health Worker-Clinical strengthens connections between patients, healthcare teams, and social service systems by providing hands-on support to people managing chronic conditions. Working primarily in a clinic environment, the role engages patients in disease-specific programs through education, coaching, and care coordination. It also identifies eligible individuals and facilitates enrollment in Remote Patient Monitoring (RPM) services for conditions such as hypertension, obesity, maternal health, and other targeted health needs. The position maintains face-to-face interaction with patients and care teams through clinic huddles, outreach, and coordination activities, and collaborates with clinical staff to track progress toward care goals. Occasional home visits help assess barriers to care, reinforce self-management skills, and ensure effective use of remote monitoring equipment. By bridging clinical care and community resources, the role supports patient engagement, reduces obstacles to care, and helps improve health outcomes across the population served.
