About The Position

The Community Health Worker-Clinical strengthens connections between patients, healthcare teams, and social service systems by providing hands-on support to people managing chronic conditions. Working primarily in a clinic environment, the role engages patients in disease-specific programs through education, coaching, and care coordination. The worker also identifies eligible individuals and helps enroll them in Remote Patient Monitoring (RPM) services for conditions such as hypertension, obesity, and maternal health. The role involves regular face-to-face interaction with patients and care teams through clinic huddles, outreach, and coordination activities. It also includes close collaboration with clinical staff to track progress toward care goals and occasional home visits to assess barriers to care, reinforce self-management skills, and support effective use of remote monitoring equipment.
