About The Position

The Community Health Worker strengthens connections between patients, healthcare teams, and social service systems by providing hands-on support to people managing chronic conditions. Working primarily in a clinic, the role engages patients in disease-specific programs through education, coaching, and care coordination. The worker identifies eligible patients and facilitates enrollment in Remote Patient Monitoring (RPM) services for conditions such as hypertension, obesity, maternal health, and other targeted health needs. The position involves regular face-to-face interaction with patients and care teams through clinic huddles, outreach, and coordination activities, and close collaboration with clinical staff to track progress toward care goals. Occasional home visits help assess barriers to care, reinforce self-management skills, and support effective use of remote monitoring equipment. By connecting clinical care with community resources, the role supports patient engagement, reduces obstacles to care, and helps improve population health outcomes. Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. Work location: Gateway Plaza. Some travel may be required for home or clinic visits.
