About The Position

The Community Health Worker strengthens connections between patients, healthcare teams, and social service systems by providing hands-on support to people managing chronic conditions. Working primarily in a clinic environment, the role engages patients in disease-specific programs through education, coaching, and care coordination. The Community Health Worker also identifies eligible individuals and facilitates enrollment in Remote Patient Monitoring (RPM) services for conditions such as hypertension, obesity, maternal health, and other targeted health needs. The position involves regular face-to-face interaction with patients and care teams through clinic huddles, outreach, and coordination activities, and collaboration with clinical staff to track progress toward care goals. Occasional home visits are conducted to assess barriers to care, reinforce self-management skills, and ensure effective use of remote monitoring equipment. By connecting clinical care with community resources, the role supports patient engagement, reduces obstacles to care, and helps improve health outcomes across the population served. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. The work location is Gateway Plaza, embedded in the Emergency Department.
