Community Health Worker

About The Position

Saint Alphonsus Health System is hiring a part-time, as-needed Community Health Worker for its mobile health clinic serving rural and underserved communities across the Treasure Valley, Western Idaho, and Eastern Oregon. The clinic provides free healthcare and vaccination services at locations such as migrant farms, homeless shelters, and community partner sites. The clinic operates similarly to an urgent care and is staffed by a provider, registered nurse, and medical assistant. The role supports marginalized populations, including people who may not speak English as their first language. The Community Health Worker serves as a liaison among the healthcare system, community organizations, and patients—especially patients with identified social needs—and connects them with local resources and support. The role includes screening for health-related social needs, documenting in real time in Epic, and following up with patients to support their ongoing health and well-being. This is a PRN position and does not qualify for benefits, but it qualifies for retirement and employee discounts. Full orientation and training are required, including in-person classes at the Boise Regional Medical Center; some flexibility may be needed to complete the training. The mobile role involves travel to clinic locations, setup and breakdown, and physical activity. Mileage reimbursement is available for travel to and from clinic sites. Clinics may run into the evening or on weekends, with pay differentials when available. Weather may affect driving or clinic conditions, and the organization may provide equipment to help mitigate conditions. The expected schedule is five mobile clinic shifts per month, with the opportunity to work up to 16 additional hours per week supporting other Community Health Worker programs and projects. Clinics typically run from 8:00 a.m. to 5:00 p.m. and rotate on Mondays, Tuesdays, and Thursdays; an occasional Saturday may be offered. Most clinics are in Canyon County, with a monthly clinic in Ontario/Baker planned and additional Boise-area locations possible. Locations may change based on community patient-care needs.
