About The Position

This role involves providing direct support to clients in a homeless shelter setting, focusing on improving their health and well-being. The Community Health Worker will act as a liaison between clients and healthcare/social service systems, assist with navigating applications and paperwork, and provide informal counseling and support. A key aspect of the role is empowering clients to understand their health conditions, develop healthier behaviors, and build social capital. The position also involves working with service systems to ensure cultural relevance and responsiveness, conducting health screenings, and collecting data for healthcare providers. Additional duties include handling emergency situations, maintaining confidentiality, and performing other tasks as assigned by the supervisor.

Requirements

  • High School Diploma/GED
  • CHW Certification preferred
  • One or more years of experience
  • Must agree with and sign LifePath’s Statement of Christian faith and be able to share that faith and knowledge with staff and guests
  • Excellent and demonstrated interpersonal relations and both written and oral communication skills
  • Proficiency with MS Office Suite, computer input, and database management
  • Ability to walk, stand and/or sit for long periods of time, reach, lift, bend, kneel, stoop, climb stairs, push, and pull items weighing 25 pounds or less; manual dexterity; auditory and visual skills; ability to follow written and oral instructions and procedures.

Responsibilities

  • Help clients understand their health condition(s) and develop strategies to improve their health and wellbeing
  • Facilitate communication and client empowerment in interactions with healthcare/social service systems
  • Serve as a liaison between health/social services and the community to facilitate access to quality services and health resources
  • Assist clients with insurance applications or other paperwork and provide informal counselling, support, and follow up with them as needed
  • Assist clients to build understanding and social capital to support healthier behaviors and lifestyle choices
  • Work with healthcare and social service systems to help them be culturally relevant and responsive to their service population
  • Provide health service screenings such as blood pressure monitoring, coaching diabetes monitoring, observe medication consumption, and medication counts
  • Collect data and report findings to healthcare providers
  • Evaluate emergency situations and handles them when the Supervisor is not in the area
  • Maintains confidentiality of all data, including guests, employee, and operational data
