About The Position

This is a part-time position for a Community Health Worker at a homeless shelter. The role involves helping clients understand their health conditions, developing strategies for improvement, and facilitating communication with healthcare and social service systems. The Community Health Worker will act as a liaison, assist with paperwork and insurance applications, provide informal counseling and support, and help clients build social capital for healthier behaviors. They will also work to make health and social services more culturally relevant and responsive. Responsibilities include conducting health screenings like blood pressure monitoring and diabetes coaching, observing medication consumption, and performing medication counts. The role also involves data collection and reporting findings to healthcare providers. Additional duties include handling emergency situations in the supervisor's absence and maintaining confidentiality of all data.

Requirements

  • High School Diploma/GED
  • CHW Certification preferred
  • One or more years of experience
  • Must agree with and sign LifePath’s Statement of Christian faith and be able to share that faith and knowledge with staff and guests
  • Excellent and demonstrated interpersonal relations and both written and oral communication skills
  • Proficiency with MS Office Suite, computer input, and database management
  • Ability to walk, stand and/or sit for long periods of time, reach, lift, bend, kneel, stoop, climb stairs, push, and pull items weighing 25 pounds or less
  • Manual dexterity
  • Auditory and visual skills
  • Ability to follow written and oral instructions and procedures
  • Excellent reading and writing skills
  • Typing/computer usage
  • Proficient interpersonal relations and communicative skills
  • Possess a valid Pennsylvania Driver’s License

Responsibilities

  • Help clients understand their health condition(s) and develop strategies to improve their health and wellbeing
  • Facilitate communication and client empowerment in interactions with healthcare/social service systems
  • Serve as a liaison between health/social services and the community to facilitate access to quality services and health resources
  • Assist clients with insurance applications or other paperwork and provide informal counselling, support, and follow up with them as needed
  • Assist clients to build understanding and social capital to support healthier behaviors and lifestyle choices
  • Work with healthcare and social service systems to help them be culturally relevant and responsive to their service population
  • Provide health service screenings such as blood pressure monitoring, coaching diabetes monitoring, observe medication consumption, and medication counts
  • Collect data and report findings to healthcare providers
  • Evaluate emergency situations and handles them when the Supervisor is not in the area
  • Maintains confidentiality of all data, including guests, employee, and operational data
