About The Position

The Community Health Worker (CHW) is a full-time field teleworker position with Aetna Better Health of West Virginia. The employee must reside in West Virginia and in one of the listed counties in the Northern region. The role is part of the care management team and involves telephonic and face-to-face member contact to assess needs, provide support, address gaps in care, and connect members with healthcare providers and community resources. CHWs serve as a bridge between members, communities, and the healthcare system through outreach, education, and advocacy. The population served includes Medicaid members of all ages, including children and youth in the Children’s Specialty program and their families. The role involves extensive field travel—70% or more of the time—in a personal vehicle to locations such as member homes, residential treatment facilities, shelters, group homes, agencies, and other community settings. Mileage is reimbursed under company policy. Standard business hours are Monday through Friday, 8 a.m. to 5 p.m. Eastern Time, with flexibility to work outside those hours as member availability requires. A flexible schedule may become available after six months, subject to demonstrated performance and attendance.
