About The Position

This position is fully grant-funded. Employment is contingent on continued grant funding. Every employee of Stockbridge Munsee Community is expected to present themselves in a professional manner to customers as well as other departments. Stockbridge Munsee Community strives to provide a provide a positive team environment where everyone contributes. Under the direction of the Community Health Outreach Director, the Community Health Worker (CHW) will serve as a community advocate and/or liaison. CHW will primarily focus on harm reduction and opioid abatement strategies. This position will provide individual and population-based health education, as well as conducting community-based health needs assessments. This position will promote and support the Harm Reduction Program and collaborate with community partners as needed.

Requirements

  • Must submit to and pass a pre-employment drug and health screening.
  • Must be at least 18 years of age, unless otherwise stated on job description.
  • Must maintain an acceptable departmental attendance record.
  • Must be able to work weekends, nights and holidays.
  • Must obtain and maintain Elder/Youth License as stated on job description.
  • Must possess High School Diploma or G.E.D. equivalent.
  • Must submit to a Criminal Investigation Background Check.
  • Must adhere to the Tribe’s Drug and Alcohol Free Workplace Policy during the course of employment.
  • Must be eligible for coverage under the employer’s liability insurance.
  • Must have demonstrated ability to maintain satisfactory working record in any prior or current employment.
  • Must be able to meet physical requirements of position.
  • Must have a valid driver’s license, reliable transportation, and insurance are required.
  • Must obtain a Wisconsin driver’s license within 30 days of employment if applicant has an out-of-state driver’s license.
  • Must meet and maintain the eligibility to operate a personal or tribal vehicle under the driver acceptability guidelines as established by Mohican Nation Insurance.
  • Must abide by departmental and organizational safety, testing, and uniform guidelines.

Nice To Haves

  • Experience with electronic health record systems is preferred.
  • Computer and mechanical aptitude preferred.
  • Should have great interpersonal communication skills.
  • Knowledge of substance use disorder and harm reduction programs is preferred.

Responsibilities

  • Provide culturally responsive and appropriate health education, information, and outreach.
  • Generates reports using Microsoft Outlook, Excel spreadsheets, Microsoft Word, Publisher and office products.
  • Collects and enters project data into required data collection systems.
  • Maintains appropriate records management systems: hard and soft.
  • Monitors, maintains and stocks public health vending machine as needed.
  • Responsible for sourcing, ordering, packaging, tracking vending machine materials.
  • Works closely with other agencies to obtain materials for vending machine.
  • Ability to establish and maintain good working relationships with individuals of varying social and cultural backgrounds.
  • Maintains awareness of the latest health recommendations and be able to advise accordingly.
  • Develop and distribute health education materials to community.
  • Coordinate care with other health-related vendors as necessary.
  • Supports operational aspects of the Community Health division to meet the organization’s customer requirements and satisfaction.
  • Required to attend job related, in-service, meetings, and training to maintain professional and technical knowledge.
