About The Position

Affinity Health Center is seeking a Community Health Worker (CHW) to join their team. This role involves being a health advocate, resource navigator, and a key participant in diabetes education and prevention efforts. The CHW will act as a bridge between patients and the care they need, providing outreach, support, and follow-up within the community. They will educate, empower, and engage individuals and families in managing and preventing diabetes, assist with diabetes education classes and individual coaching using plain language and culturally relevant materials, and collaborate with nurses and healthcare providers to help patients build healthier habits. The role also involves guiding patients through health systems and accessing community resources, building trust, fostering empowerment, and being a consistent, caring presence in patients’ health journeys.

Requirements

  • High School diploma or GED
  • Completion of a Community Health Worker training program or willingness to be trained
  • Experience working with underserved populations
  • Strong knowledge of and relationships within the local community to be served through this program.
  • Must have valid driver’s license and reliable transportation (home visits and community outreach are a part of the role)

Nice To Haves

  • Associate’s degree
  • Certified Community Health Worker
  • Experience with diabetes or chronic disease management
  • Bilingual (especially Spanish)
  • Familiarity with diabetes, nutrition, or chronic disease education
