About The Position

Ozarks Community Hospital (OCH) in Rogersville, MO is seeking a Community Health Worker (CHW) for their Medical Home department. The CHW will be a key member of the client resource team, connecting patients with resources to address social determinants of health that affect their well-being. Responsibilities include providing informal coaching, counseling, social support, culturally appropriate health education, and basic screening services, all while adhering to the standards, goals, and philosophy of OCH and its Employee Code of Conduct.

Requirements

• High school diploma or equivalent.
• Must obtain CHW certification through an approved curriculum within 12 months of hire date.
• Valid Driver's License required.
• Must have a valid Class E license upon hire or Class E license must be obtained during orientation period.
• Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class.
• Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
