Requirements

Responsibilities

  • Conduct community outreach and connect individuals with appropriate health and social-service resources.
  • Use an approved screening tool to identify needs related to food, housing, transportation, healthcare, and economic stability.
  • Provide warm handoffs to service providers and document all referrals and follow-up activities.
  • Manage a caseload of community members participating in health and wellness programs.
  • Help clients develop goals, understand care plans, manage chronic conditions, and overcome barriers to care.
  • Coordinate communication among clients, healthcare providers, community organizations, and internal teams.
  • Maintain accurate electronic client records, case notes, and monthly reports.
  • Support evidence-based chronic-disease prevention programs.
  • Serve at Y food pantries and support Y Fresh Mart and other food-access initiatives.
  • Participate in Y on the Fly activities, quarterly Community Health events, and West Baltimore outreach events.
  • Travel to client homes, clinics, Y locations, agencies, and community events.
  • Model the Y’s core values of caring, honesty, respect, and responsibility.
