About The Position

The Lead Community Health Worker (LCHW) serves as the key liaison between the healthcare system and the community, leading efforts to improve health outcomes by connecting individuals and families with health services, resources, and educational opportunities. The LCHW will provide support and coaching to a team of Community Health Workers (CHWs), provide input in program development and learning curriculum for CHW growth, while actively engaging with local populations to address social determinants of health, reduce health disparities, and promote health equity. LCHW will carry a modified caseload of clients. Home and community-based visits is work assignment specific but could be an essential function of a CHW's work. Travel to various sites within Jefferson enterprise to support other CHWs is required. Location assignment and work settings could include: outpatient care areas, patient homes, inpatient settings, skilled nursing, community organizations, and participating in community events. Participation in community events and some evening/weekend hours may be required.

Requirements

  • working as a CHW in healthcare or a community based organization required
  • working with people who have complex care needs required
  • Provides excellent and consistent service to patients/clients/families, colleagues, community members and community partners
  • Demonstrates strong ability to coach and mentor CHWs by breaking down unfamiliar topics to make them manageable for CHWs to develop their awareness of health conditions and health care system; builds up confidence with coordination of services, tracking of patient/client/family progress on goals and resource navigation; and can educate on healthy lifestyle habits
  • Understands various community cultures and ways to engage them so they feel seen, heard, and empowered
  • Excellent communication skills and can de-escalate and resolve conflicts when they arise
  • Follow ethical standards like codes of ethics, laws, and institutional guidelines

  • Willingness to conduct home visits (may not be applicable for all CHW location assignments)

Responsibilities

  • Train new Community Health Workers (CHWs) and provides ongoing staff development instruction while serving as a mentor and coach to CHWs in the areas of resource navigation, adopting healthy lifestyle behaviors, and navigating health care systems
  • Serve as subject matter expert and trainer on Health Related Social Needs (HRSN)/Social Needs assessments and workflow in the electronic health record (EHR) and remain knowledgeable of appropriate community resources to support clients and CHW resource navigation
  • Collaborate with Managers and Director to develop trainings, improve workflows, creation of training modules and continuing education curriculum, and identify and resolve problems
  • Coordinate day of oversight and provide CHW support for community events
  • Carry a modified client caseload at the direction of supervisor and/or director and complete accurate, timely documentation
  • Apply cultural humility and diversity, equity, and inclusion principles when interacting with clients and staff and demonstrates clear communication and positive interpersonal skills
