About The Position
Scheurer Health-Bad Axe is seeking a full-time Community Health Worker (CHW) to join our team. The CHW will work in both Scheurer Physician Clinics and School Based Clinics.
Requirements
- High School Diploma or G.E.D Equivalent
- Two years of experience in a healthcare-related field
- For the school-based clinic, experience working with children, adolescents, families, and schools
- Community Health Worker certification, if not currently certified, you must certify within six months of hire
- Be able to interact with all age groups
Responsibilities
- Provide outreach, care coordination, resource navigation, and health education to support students and their families
- Identify and address barriers that my impact students and families
- Connect individuals to appropriate healthcare, behavioral health, social services, and other community partners
